身近な致死性人獣共通感染症「重症熱性血小板減少症候群」に関する研究内容

2011年に中国から「重症熱性血小板減少症候群」Sever Fever with thrombocytopenia syndrome; SFTS) というマダニ媒介性ウイルス感染症が新たに報告されました。クリミア・コンゴ出血熱などと同じブニヤウイルスのグループで、ダビエバンダウイルスと新たな分類名がつけられました(ここでは便宜上SFTSウイルスとします）。日本においても2013年の山口県での致死例が初めて報告されて以来、西日本を中心に、毎年70人以上が発症しています。特に宮崎は全国でも患者発生数が高い地域となっています。症状は、発熱、下痢を中心とする消化器症状を経て、強度の倦怠感、筋肉痛、意識障害、皮下出血や下血などの出血症状が報告され、その致死率は15-25%であり、国内における感染症としては非常に高い数値となっています。

SFTSはマダニによって媒介されるとして認識されており、マダニによる動物への吸血によりウイルス感染環が維持されていると考えられています。我々も宮崎のマダニからSFTSウイルスを分離し確認しています（Sato et al., 2020)。しかし、2017年以降に、衰弱したネコを保護しようとして手を噛まれた女性(50歳代)が、SFTSを発症し亡くなり、ネコからヒトに感染するという可能性が考えられました。

我々は、獣医療者がSFTS発症ネコを診療したのちにSFTSを発症するという事例を明らかにし報告しました。我々の追跡調査により、発症ネコおよび獣医療者から分離されたウイルスの遺伝子配列が一致したことから、同一ウイルスによる動物病院における院内感染事例が明らかとなりました(Yamanaka et al., 2020)。

この結果を受けて、SFTS流行地宮崎におけるSFTSウイルスに対する抗体調査を行ったところ、獣医療者の抗体保有率が一般の方よりも高い傾向があることから、獣医療者への感染リスクがあるという注意喚起をしています(Kirino et la., 2021)

牛の風邪はウイルスと細菌が関係している？「牛呼吸器症候群」に関する研究内容

牛呼吸器症候群は、ウイルスおよび細菌等の病原微生物と免疫状態のバランスによって発生し、牛産業界で最も経済的損失の大きな疾病として知られている。我々は、BRDCのウイルス要因の一つであるウシRSウイルスが下部気道由来の牛呼吸器上皮細胞に感染した際に、パスツレラの細胞表面への付着を亢進することによって、下部気道における炎症を増強することを確認している。ウイルス感染がICAM1やPAF-Rといった細胞表面上の接着因子の発現を調整し、細菌の付着性やその後の炎症を誘導し、重症化に導いていることを明らかにしている (Sudaryatma et al., 2020)。
